1. Instructions for Trainee DVDs

Upon registration you will receive packs (research packages) from Imperial College, London which contain:

  1. the relevant consent forms for both you and your patient which should be completed prior to the case starting,
  2. a hard copy of GAS forms, Form A and B and
  3. a blank DVD.

This DVD should be used to record each case, and returned with the relevant completed forms (i.e. both consent forms and also Form A and B if these are not submitted electronically on-line) to Imperial College. This video data is key for the research that is being carried out by Imperial College into LAPCO on behalf of the Department of Health. Hence, all trainers and trainees should take active part in data collection.

A stamped addressed envelope is included in each pack, with the return address: Melody Ni, Imperial College, Dept of Biosurgery, 10th floor, QEQM Building, St. Mary's Hospital, London W2 1NY) . Once received by Imperial College the DVDs and forms will be anonymised before they are sent to the researchers for analysis.

Blank DVDs will be supplied by Imperial. Please contact Melody if you require more DVDs.

2. Instructions for "Signing Off" DVDs

Imperial College will contact the trainee once a recommendation has been made for "sign off".  For this process two DVDs are required from the trainee and should be:

Full length, i.e. non-edited

Two different cases e.g. one right sided resection and one left sided resection

From operations done without supervision.

Each DVD needs to be labelled clearly with your name and the type of operation, and sent to Melody Ni at Imperial College, London (see above for contact details).  Upon receipt, the DVDs will be made anonymous and passed on to two NTP assessors. The National Clinical Lead will advise the trainee and training centre in writing of the feedback and recommendation for "sign off".

3.  Instructions for Trainer DVDs

DVDs for the web site should be approximately 10 minutes long, they need to be labelled clearly with the trainers name and operation being performed.

DVDs produced by Trainers that need to be uploaded to the website should be sent to the team at Yeovil District Hospital for editing:

Upon completion of editing the DVD will be sent to the Coordination Office who will authorise the approved material to be uploaded to the web site library. 

If the DVD has already been edited to the length required, send it directly to the Coordination Office for approval by the National Clinical Lead before it is uploaded to the web site.

4.  Format for DVDs

All DVDs should be clearly referred with the trainer/trainees name on the box, and the DVD itself.

DVDs should be sent in one of the following formats:

Quick time
MPEG
MOV
FLV
AVI
